One in four American adults is now a family caregiver. Yet too many are navigating complex care needs alone.
The National Association of Community Aging Circles (NACAC) empowers families and communities with the tools, support, and connections needed to help older and vulnerable individuals age safely at home—while staying connected to the people who matter most.

Caregiving Shouldn’t Feel This Overwhelming
Families today are taking on more responsibility than ever—often without guidance, coordination, or support.

How It Works
Build a “Circle of Care” Around Every Individual
At the heart of our model is the Circle of Care—a coordinated network of people and services supporting one individual.
Step 1: Connect
We partner with community organizations to identify and support families in need.
Step 2: Organize
Families create a personalized care circle that may include: Family members Friends and neighbours Healthcare providers Community services (e.g. meals, transport, home aides)
Step 3: Coordinate
Using easy-to-use digital tools, everyone in the circle can: Communicate in one place Track care tasks and responsibilities Share updates in real time
Step 4: Support
NACAC provides training, resources, and ongoing guidance to ensure families feel confident and supported.
